Personal care is often the support that makes independent living possible. Our team assists with showering and bathing, dressing and grooming, toileting and continence support, help with mobility and transfers, and assistance with eating and drinking. We also support medication prompting and monitoring where it forms part of your plan.

Every person's routine is different, so we build the support around how you actually like to do things — not a standard checklist. That includes the time of day you prefer, the level of help you want versus the independence you want to keep, and any cultural or personal preferences around who provides your care.

Because Jade Care was founded by two registered nurses, our personal care team is trained and supervised with clinical oversight. That matters when personal care overlaps with health needs like skin integrity, pressure care, or changing mobility.

Community nursing brings clinical care into your home, so you don't have to travel to a clinic for supports that can safely be delivered where you live.

Our registered nurses provide medication management and administration, wound care and dressing changes, continence assessment and management, health monitoring and observations, diabetes support including blood glucose monitoring, and PEG and enteral feeding support. We also carry out clinical assessments and can work alongside your GP, specialists and allied health team to keep everyone aligned on your care.

This is the service where our background matters most. Jade Care was founded by two registered nurses, and clinical governance sits at the centre of how we operate rather than being added on. For participants with complex or high-intensity needs, that means care delivered and supervised by people who understand the clinical picture, not just the support schedule.

NDIS Funding

Typically funded under: Core Supports, or Improved Health and Wellbeing, depending on your plan and whether the support is classed as high intensity.

Supported Independent Living (SIL) is help with the everyday tasks of running a home when you live with others — usually one to three housemates. The goal is a real home life with the right level of support, not supervision for its own sake.

Our support workers assist with meal planning, cooking and grocery shopping, cleaning and laundry, household routines and budgeting, personal care where required, medication support, and getting to appointments and activities. We also help with the social side of shared living: navigating housemate relationships, agreeing on shared responsibilities, and building the skills to do more independently over time.

Support can be scheduled through the day, overnight with active support, or overnight with sleepover support, depending on what your plan funds and what you actually need. We work with you, your family and your support coordinator to get that balance right, and revisit it as your independence grows.

Community participation is about having a life outside the house — and having someone alongside you to make that possible.

Our support workers help you take part in the things you're actually interested in: joining a local club or class, attending community and cultural events, going to the gym, pool or park, visiting the library or shops, volunteering, or simply meeting friends for coffee. Where you're not sure what's out there, we help you explore options and try things without pressure to commit.

Support is tailored to how much you want. For some participants that means one-to-one accompaniment and help managing a new environment. For others it's a gradual step back — being there for the first few visits, then less, as confidence builds.

Because our team speaks Mandarin, Taiwanese, Korean, Hindi, Punjabi and English, we can also support participants to stay connected to their own cultural communities, events and places of worship, in a language they're comfortable in.

A home that's clean, organised and safe makes everything else easier. Our team helps with general and deep cleaning, laundry and ironing, changing bed linen, dishes and kitchen tidying, grocery shopping and putting shopping away, meal preparation, rubbish and recycling, and light garden or outdoor upkeep.

We can do these tasks for you, or do them alongside you — whichever suits your goals. Many participants use household support partly as skill building: cooking together, working out a cleaning routine that's realistic, or learning to shop to a budget. If that's what you want, tell us and we'll work that way.

Support can be regular weekly visits, a set of tasks on a schedule, or extra help at particular times such as after a hospital stay or during a period of poor health.

Group activities give participants a regular social outlet and the chance to build genuine friendships, with support staff present the whole time.

Our groups include day outings to parks, beaches and markets, cafe and restaurant meetups, movies and entertainment, sport and recreation, arts, crafts and music sessions, and seasonal or cultural celebrations. Group sizes are kept manageable so everyone gets attention and nobody gets lost in the crowd.

For participants who find one-to-one support isolating, or families looking for their loved one to build a social circle, groups often do what individual support can't. They also tend to stretch a plan further, since the cost of support is shared across the group.

We match participants to groups based on interests, age and support needs, so the mix works socially rather than just logistically. If you're not sure whether a group is right, you're welcome to come along to one and see.

Life skills support is about doing more for yourself over time. Rather than a service that does things for you, this is structured, patient teaching aimed at independence.

We support participants to build skills in cooking and meal planning, budgeting and money handling, using public transport and navigating safely, personal care routines, cleaning and home maintenance, using phones, computers and the internet, communication and self-advocacy, and managing appointments and personal admin.

We start by working out what you actually want to be able to do, break it into steps that are achievable, and practise them in real settings — a real kitchen, a real bus route, a real shop. Progress is reviewed regularly with you and, where you want, your family or support coordinator, so everyone can see what's changing.

The pace is set by you. Some skills come together in weeks, others take much longer, and both are fine.

Getting there is often the barrier. We provide safe, reliable transport to medical and allied health appointments, work and study, day programs and activities, shopping and errands, social visits and community events, and family occasions.

Support goes beyond the drive. Our workers can accompany you into an appointment, help you communicate with staff, wait with you, and make sure you get home settled. For participants working towards independent travel, we can also do this as training — travelling with you on public transport, practising the route, then stepping back as you take it on yourself.

Transport is coordinated around your schedule and can be combined with other supports in the same visit, which usually makes better use of your plan than booking them separately.

Typically funded under: Core Supports — Transport, or as part of the support worker's time depending on how your plan is structured.

An NDIS plan is only as good as your ability to use it. Support coordination is the help that turns a funded plan into services actually running in your life.

Your support coordinator helps you understand what's in your plan and what each budget can be used for, find and compare providers, set up service agreements and bookings, resolve problems when a service isn't working, coordinate between your providers so nothing falls through the gaps, build your confidence to manage more of this yourself over time, and prepare evidence and reporting for your plan review.

We work with participants on all plan management types — Plan Managed, Self Managed and NDIA Managed — and we'll be straight with you about what your plan can and can't cover.

If your plan has just started, has changed significantly, or simply isn't being used, this is usually the first service worth putting in place.
